Forward head posture (FHP) is characterized by increased flexion of the lower
cervical spine and upper thoracic region and increased extension of the upper
cervical spine (1). FHP is commonly defined as the protrusion of the head in the
sagittal plane so that the head is placed anterior to the trunk (2). It can occur
because of anterior translation of the head, lower cervical flexion, or both, and it is
claimed to be associated with an increase in upper cervical extension (3), (4). A
knowledge of the bony configuration, ligamentous attachments, joint
articulations, vascular supply, muscle function, and lymphatic drainage as well as
the kinetic anatomy of the cranio-cervical junction (CCJ) is necessary to
understand the etiology of abnormalities in this area and their treatment. The
craniovertebral junction (CVJ) is the most mobile of the upper cervical spine. It is
uniquely adapted for stability and motion (5).
